How to Attach Follow-Up Notes to an Appointment

Here is how you attach a  Form Template to be made available to your client after an appointment has been closed.  This works well for follow-up notes, home-care instructions or as a follow-up survey of their appointment.

Create the form template (or follow-up instructions or survey) in your form templates, then attach that form to the service.

On the Done Appointment window, All the forms that are assigned to this appointment with a checkbox, if the checkbox for each of the forms is checked, when the appointment is saved as Done, the client will receive an email with a link to be able to view the form.
