How to Attach Forms to a Service (Required Intake Forms)

How to set up a required Intake Form

This is for when you would like your client to fill out your intake form before arriving at your business for their service.  You do this by:

  1.  Set up a form template and save it to your Form Templates.
  2. In the Advanced Configurations of your Form, select the Services you require this form to be filled out for, and also if your clients will need to fill it out every time they book an appointment, or only the first time.
  3. Need to add a new Service to your list?  Go to Services to add or edit your service list.

When these services are booked, your client will be sent an email with a link to the form.  They will be able to fill it out immediately or by logging into their account as seeing it in their list of upcoming appointments.
